1. Please complete the form and submit online. Signatures can be added later. A PDF version is also available online. 
2.The appropriate fee (by local cheque or demand draft payable at Chennai) along with the Rules  and three stamp size coloured photos, should be sent to our postal address.
3. Acknowledgement of this form does not amount to enrolment as a member.
4. Two persons may be nominated to the Club for each Company Membership.
5. Membership cards will be sent after your application is accepted.

Individual Life Membership Rs. 10,000 (No entrance fee)
Company Life Membership Rs. 20,000 (No entrance fee)
(Two nominees allowed)
Individual Annual Membership Rs. 2,000 per year
and entrance fee Rs. 4,000 (one time)

Associate Annual Membership
(No voting rights and cannot hold
any office in the club)		 Rs. 500 per year
and entrance fee Rs. 1,000 (one time)
Outstation Annual membership
(No voting rights and cannot hold
any office in the club)		 Rs. 500 per year
and entrance fee Rs. 500 (one time)
